PURSUIT PLAYER REGISTRATION FORMS

2008 - 2009 Pursuit Player Package

Every Pursuit player is required to fill out the Pursuit Player Package. The package includes the  "Player and Parent Contract", "Player Standards of Conduct", "Permission to Use Likeness" and "Contact information Sheet". This information is retained by the coaches and all new players are required to fill out these forms. It is recommended that each player re-fill out this paperwork at the beginning of each Fall Season. The Agreement of Financial Responsibility is required to be completed each year. 

 

2008 - 2009 Medical Release Form

The "Medical Release" form is for parents to certify permission for their daughter's participation in any Pursuit Soccer Club activities. Every tournament that is entered needs to see this form for each player.  Although STYSA does not require the form to be notarized, most doctors will not treat a child without a notarized form, should the child's guardian not happen to be at a game. Pursuit strongly recommends you use the form with required notary signature attached.

 

Supplemental Insurance Claim Form

Use this form to file your supplemental claim.  Benefits will be paid first by other valid coverage, that is, your primary medical insurance and the STYSA policy.  This policy is essentially to cover deductible out of pocket expenses from your primary insurance.  The insurance we provide along with the STYSA insurance will coordinate benefits with the parent's primary coverage (the STYSA insurance will act as primary in the event a parent doesn't have coverage, up to the policy limits).

 

Individual Player Registration Form

This form is required to be filled out by every player on your roster. It is turned in to EDDOA along with your roster, Adult Registration Forms and Team Registration Form in the Fall. It will also be needed in the Spring if you are playing National or President's Cup and you are adding a new player.
